Telescribing

Telemedicine has become part of our "new normal," so it makes sense that telescribing should be too.  Technology tools are now more widely available whereby scribes can observe the patient encounter either by joining a telemedicine appointment or via webcam rather than being physically present.  Most EHRs allow for remote access, so the scribe can document the encounter in the usual fashion, contemporaneously with care.  Some "virtual scribe" offerings are actually a recording of the visit, that is transcribed later via store-and-forward technology.  While this approach can offer some cost savings, the real-time working relationship between the provider and scribe is lost, and notes are not completed real-time.  

Telescribing can have major advantages not only in times of an infectious pandemic, but also for clinics simply suffering from cramped quarters or limited parking.  Providers in rural areas often cannot find good scribes unless they are near a college or other educational center, but with telescribing, we can recruit and deploy scribes from anywhere.

  • Review article found that the available evidence suggests scribes may improve clinician satisfaction, productivity, time-related efficiencies, revenue, and patient-clinician interactions.

  • Scribes had a positive impact on RVUs in adult but not pediatric patients.  For adults, scribed visits produced 0.20 additional RVUs per patient (p < 0.001).

  • There was a 36% reduction in time spent documenting and a 30% increase in time spent in direct patient contact.    RVUs per hour increased 5.5% and per patient 5.3%.

  • Scribes improved all aspects of physician satisfaction, including overall satisfaction with clinic (OR = 10.75), having enough face time with patients (OR = 3.71), time spent charting (OR = 86.09), chart quality (OR = 7.25), and chart accuracy (OR = 4.61). 
  • Scribes increased the proportion of charts that were closed within 48 hours (OR =1.18, P =.028).

  • In this crossover study of 18 primary care physicians, use of scribes was associated with significant reductions in electronic health record documentation time and significant improvements in productivity and job satisfaction.

  • Patients seen per hour increased by 59% and RVU per hour increased 57%.   Direct and indirect (downstream) revenue per patient seen was $142 and $2,398, with $205,740 additional revenue generated from the 81 additional patients seen with scribes.
